Application of Magnetic Resonance Imaging (NRI) in the diagnosis and assessment of diffuse axonal injury

  • 摘要: 目的 评价常规MRI不同序列与DWI在DAI诊断和评价中的价值.方法 对34例临床诊断为DAI的患者行常规SE-T1WI、FSE-T2WI、FLAIR-T2WI、平面回波弥散加权(DWI)成像和T2加权梯度回波序列T2WI/FFE扫描,比较各系列脑内病变显示率,分析其信号特征.结果 34例患者总计检出246个病灶,其中出血性病灶89个.在DWI上检出182个病灶,包括高信号病灶161个;15个高信号灶和21个低信号灶为出血性病灶.67.1%的高信号病灶ADC值降低, 21.2%的ADC值增高, 11.7%的ADC值正常;12.6%病灶在其它序列上未能检出.FLAIR-T2WI检出153个病灶, FSE-T2WI检出131个病灶;T2WI/FFE检出112个病灶, 89个为出血灶;其中53个在其它序列上未显示.对DAI病灶以DWI的检出率最高,其次为FLAIR-T2WI.对出血性病灶以T2WI/FFE为最高,但该序列对非出血灶检出率很低,与其它3个序列相比具有显著差异.结论 DAI的诊断和评估具有重要的临床意义, DWI检出DAI病灶的总敏感性最高, T2WI/FFE在检出出血性DAI病灶上具有优势.DWI、FLAIR/FSE-T2WI和T2WI/FFE结合应用能够较全面的对DAI作出诊断和评价.
