Therapeutic effects of ambroxol hydrochloride combined with glucocorticoids in the treatment of idiopathic pulmonary fibrosis

摘要: 目的 观察盐酸氨溴索联合糖皮质激素治疗特发性肺间质纤维化(IPF)的疗效。 方法 选取100例IPF患者为研究对象,按照随机数字表法分为对照组和观察组,每组50例。对照组应用糖皮质激素治疗,观察组在对照组的基础上应用盐酸氨溴索治疗,比较2组临床疗效、肺功能[肺活量(VC)、用力肺活量(FVC)、用力呼气量(FEV)、一氧化碳弥散量(DLCO)]、血气指标如动脉血氧分压[pa(O2)]、动脉血二氧化碳分压[pa(CO2)]、血清炎症因子[白细胞介素-10(IL-10)、白细胞介素-13(IL-13)、转化生长因子-β1(TGF-β1)]水平及不良反应发生率。 结果 观察组总有效率为90.00%, 显著高于对照组的72.00%(P<0.05); 治疗后,观察组VC、用力呼气量与用力肺活量比值(FEV/FVC)、DLCO均显著高于对照组(P<0.05); 观察组pa(O2)显著高于对照组, pa(CO2)显著低于对照组(P<0.05); 治疗后观察组血清IL-10、IL-13、TGF-β1水平均显著低于对照组(P<0.05); 观察组不良反应发生率为8.00%, 显著低于对照组的24.00%(P<0.05)。 结论 盐酸氨溴索联合糖皮质激素治疗IPF的疗效显著,能够改善呼吸功能,降低炎性因子水平。Abstract: Objective To observe the effect of ambroxol hydrochloride combined with glucocorticoid in treatment of idiopathic pulmonary interstitial fibrosis(IPF). Methods A total of 100 patients with IPF were selected as subjects and divided into two groups according to the random number table method, with 50 cases in each group. Control group was treated with glucocorticoids, while observation group was treated with ambroxol hydrochloride combined with glucocorticoids. The clinical curative effect, lung function [vital capacity(VC), forced vital capacity(FVC), forced expiratory volume(FEV), diffusion lung capacity for carbon monoxide(DLCO)], levels of blood gas indicators such as arterial partial pressure of oxygen [pa(O2)], partial pressure of carbon dioxide in arterial blood [pa(CO2)], serum inflammatory factors [interleukin-10(IL-10), interleukin-13(IL-13), transforming growth factor-β1(TGF-β1)] and the incidence of adverse reactions were compared between the two groups. Results The total effectiverate of the observation group was 90.00%, which was significantly higher than 72.00% of the control group(P<0.05). After treatment, the VC, ratio of forced expiratory volume to forced vital capacity(FEV/FVC)and DLCO in the observation group were significantly higher than those in the control group(P<0.05). The pa(O2)in the observation group was significantly higher than that in the control group, and the pa(CO2)was significantly lower than that in the control group(P<0.05). Levels of serum IL-10, IL-13 and TGF-β1 in the observation group after treatment were significantly lower than those in the control group(P<0.05). The incidence of adverse reactions in the observation group was 8.00%, which was significantly lower - than 24.00% in the control group(P<0.05). Conclusion Ambroxol hydrochloride combined with glucocorticoids is effective in the treatment of IPF, which can improve respiratory function and reduce levels of inflammatory factors.
