Clinical efficacy analysis of different audiometric curves in sudden sensorineural hearing loss patients

摘要: 目的 分析不同听力曲线类型突发性聋患者的临床特点。 方法 回顾性分析186例(200耳)突发性聋患者的临床资料,根据听力曲线类型的不同将200耳分为4组,分别对其临床特征进行综合分析,并比较性别、侧别、伴发症状(耳闷、耳鸣、眩晕)、听力损失程度以及疗效分布特点。 结果 186例患者(200耳)中,男115耳,女85耳; 左耳发病76耳,右耳发病96耳,双耳28耳; 伴耳鸣者191耳,伴眩晕者60耳; 低频下降组12耳,高频下降组23耳,平坦下降组63耳,全聋组102耳。200耳的治疗总有效率为36.50%, 低频下降组总有效率为41.67%, 高频下降组总有效率为30.43%, 平坦下降组总有效率为46.03%, 全聋组总有效率为31.37%。4组在性别、疗效、耳闷、耳鸣方面比较,差异无统计学意义(P>0.05), 在侧别、听力损失程度、眩晕方面比较,差异有统计学意义(P<0.01)。4组治疗后的听力水平较治疗前有不同程度改善。 结论 不同听力曲线类型的突发性聋发病机理不同,疗效也有不同。突发性聋患者的听力曲线以全聋型最多,低频下降型最少,其中平坦下降型疗效最好,低频下降型、全聋型次之,高频下降型最差,分型治疗有助于提供个性化的诊疗方案,为听力损失的预防及干预提供参考。Abstract: Objective To analyze characteristics of different audiometric curves in sudden sensorineural hearing loss(SSNHL). Methods Retrospective analyze of 186 cases(200 ears)of SSNHL was performed, and were divided into four groups according to different audiometric curves. The clinical characteristics of these patients were analyzed, and their gender, deaf side, concomitant symptoms(ear tightness, tinnitus, vertigo), hearing loss degree and therapeutic effect distribution were compared. Results Among 186 cases(200 ears), there were 115 ears in males and 85 in females; 76 left ear, 96 right ears, and 28 bilateral ears; 191 ears complicating with tinnitus, 60 ears with vertigo; 12 ears in low-frequency descending group, 23 ears high-frequency descending group, 63 ears in flat descending group, and 102 ears in deaf group. After the treatment, the overall effective rate in 200 ears was 36.50%, with effective rate of 41.67%, 30.43%, 46.03%, and 31.37%, respectively, in low-frequency descending group, high-frequency descending group, flat descending group and deaf group. Gender, ear fullness, tinnitus, and curative effect showed no significant differences(P>0.05), but hearing loss side, hearing loss degree, as well as vertigo showed significant difference(P<0.01). Compared with pre-treatment, hearing was improved post-treatment. Conclusion Different types of hearing curve have different pathogenesis and therapeutic effect. The hearing curve of patients with sudden deafness is the most in total deafness type, and the - lowest in low-frequency decline type, of which flat decline type has the best efficacy, followed by low-frequency decline type and total deafness type, and high-frequency decline is the worst. The classification treatment is helpful to provide personalized diagnosis and treatment scheme, and provide reference for the prevention and intervention of hearing loss.
