结肠成形术应用现状及研究进展

摘要

摘要: 随着吻合器在临床的广泛应用,中低位直肠癌患者行保肛手术的比例逐年增加,但传统的结肠断端与下段直肠或肛管作直接端端吻合术后,患者容易发生便频、便急、控便能力差、大便失禁等排便功能障碍,即"前切除术后综合征"[1],严重困扰着患者术后的生活质量.因此,尽快改善低位直肠癌患者前切术后排便功能和提高患者生活质量显得尤为重要.十多年前结肠成形术率先在国外展开,可显著改善患者术后的排便功能障碍,临床效果较为满意.Lazorthes、Parc先后构建了结肠J型储袋来改善低位直肠癌保肛手术后排便功能,取得了良好的疗效,但也存在术后排空障碍等并发症.1997年,Z'graggen提出一种新型结肠储袋术,即结肠成形术,该手术方法操作简单,临床应用效果满意.本文对结肠成形术在国内外十多年的研究进行综述,为临床治疗中低位直肠癌选择合适的手术方式提供理论依据.
